 clacking sound from the rear

can anyone tell me what is going on
I have a clack sound from the rear axle after shifting from park to drive and accelerate there is a clack sound which is loud and happens once until I put it in reverse and makes that sound again .
So far I have changed the brake shoes and loosen the hand brake but
it still makes the sound and it is doing my head in .
I'm starting to think it could be the rear axle shaft or diff or maybe its the little parts in the rear hub below of the brake shoes

It also may be the centre bearing on the tailshaft,they make all sorts of wierd noises under load.
